Hazardous and Industrial Waste Permits: Mailing List Specifications
With the exception of Class 1 or Class 11
modifications, all permit applicants are required to submit a list
of names and mailing addresses of all persons specified in 30 TAC §39.13
with their permit application. The chief
clerk of the TCEQ is required to use this list to mail the public
notice. The list must be formatted as
detailed on this page and may be submitted on a disc (preferred) or as 4 sets of printed labels on
sheets that have 30 labels to a page. The list should be updated
before any required public notice is mailed.
Format Example
Each letter in the name and address must be capitalized, contain no punctuation, and the appropriate two-character abbreviation must be used for the state. Each entity listed must be blocked and spaced consecutively as shown below. This format is required by the U.S. Postal Service for machine readability. The example below shows the layout of a 30 label per sheet format for five mailing addresses.

Mailing List on Disc
The mailing list may be submitted on a formatted compact disc using software compatible with Microsoft Office Word format, as allowed by 30 TAC §39.5(b).
Please label the disc with the applicant's name and permit number. Within the file stored on the disk, type the permit number and applicant's name on the top line before typing the addresses (see example).
A list submitted on disc should be the only item on that disc. Please do not submit a list on a disc that includes maps or other materials submitted with your application.
